Bloodworks Northwest simply wouldn’t exist without our donors. The lifesaving impact each and every donor makes on the community is nothing short of profound. Bloodworks proudly celebrates our donors, especially those who reach significant milestones with their lifesaving donations. A Milestone Donor is anyone who has voluntarily given 100, 200, 300 or more red cell, platelet and plasma units.

Be Recognized
Milestone Donors who wish to be recognized can complete the Request Form included in the Donor Recognition Package you will receive and mail it to Bloodworks, or you can complete and submit the form below.
You may also download and print the form here. Please email your completed form to [email protected].
PLEASE NOTE that you must be a Milestone Donor in order to request and that requesting a your name to be displayed is completely optional. Your name should be displayed about 2-3 months after your Request Form is submitted.
